			<content:encoded><![CDATA[<p>There is so much opinion floating out there as to the most effective solution to deal with the rising cost of our healthcare system. The debate has been on both sides of the table and has swung both directions for both Democrats and Republicans. Neither group can seem to see eye-t0-eye on how to fix the major problems inherent in the healthcare system. Democrats seem bent on making healthcare public. Republicans&#8211;for the most part&#8211;seem content with making the healthcare system private. However there are inherent risks with both methods. If you are <a title="banking" href="http://www.bankingstrategies.org/">banking</a> on the costs of healthcare decreasing over the next ten to fifteen years, I would consider you a loony. Some of the issues with both proposals are discussed below.</p>
<p>The private method holds the risk that the outpacing costs of healthcare over GDP and real wage increases will make people more and more poor in real dollars. As the cost of insurance premiums continue to rise to keep up with the cost of the healthcare itself, people will have less money to spend on other goods and services, thus decreasing the money flow and spending power of those individuals.</p>
<p>On the public side of the argument, you have democrats claiming the making healthcare for everyone will decrease costs because of pooling. Some argue the other direction, claiming that those not in the system now are not in because they can&#8217;t afford. Once they enter, they will use it more and more actually causing the costs increase substantially more than they did previously. There are a host of other social and moral arguments on both sides of the spectrum that have yet to be played out. Perhaps only time will tell.</p>
<p>It is because of the rising cost of healthcare that the Obama administration has put the Affordable Care Act in place and is causing each individual state to set its own <a title="health insurance exchange" href="http://myhix.org/">health insurance exchange</a> program or HIX. Some are refusing to get their set up. In that case, the federal government is actually planning on usurping the states and helping to set up a federal HIX for the areas that fail to set one up themselves. Will the costs continue to rise? Some say no, but my guess is that we are just shifting the rising costs to the public in a different way rather than having privatized the insurance, we have made it public. The only problem I see is the inefficiency of government bureaucracy in dealing with the whole mess will be more costly than the private version.</p>

The idea that zero defections and lost customers will produce profitability increases overtime is invariably true. However, a goal of zero defects in your customer base is a daunting task at best as customers have almost been preconditioned to switch based on cost and variety. And, as channels increase, the expanded flavors for procurement may help drive away those once loyal. In addition, buyers’ detachment from any one particular channel of distribution creates in them a somewhat distanced sense of loyalty to any one particular product. Even the most loyal customers may be difficult to corral in order to get them to “re-up” for another dose of business from your company. This is especially true for a commodity like <a title="commercial electricity" href="http://www.amenergy.com/commercial/">commercial electricity</a>.</p>
<p>Sensing the need to retain customer loyalty may drive business owners to promote programs for retention, but such efforts may also be thwarted by the status quo, the products and services they provide, the distribution channels, or even the defection management itself. Certainly defection management costs must have increased parallel with increases in channel options. If this is even remotely true, then the increase in retention costs may need re-addressed. Moreover, if the defection more readily occurs as channel increases, then the cost/revenue comparison of retention versus new client <a title="business acquisition" href="http://soltre.com/mergers-and-acquisitions/">acquisition</a> per customer may not be as appealing as it was in times past.</p>
<p>The internet has served to help in the increase of channel number and variety, further exacerbating defection away from one particular brand offering. And while internet businesses often have the freedom of lower costs of acquisition, their lack of channel variety lessens their ability to retain customers and manage defection. For instance, can the “pure-play”<a title="internet company" href="http://www.interpacket.com/"> internet company </a>reach the man or woman who needs a razor immediately in order to make his/her meeting tomorrow morning? In addition, the internet as a channel sees behavior segments which would have been considered abnormal for particular customer-types if they had not been shopping <a title="online file backup" href="http://www.sarcoidosisresearch.org/2011/online-medical-file-backup/">online</a>. Buyers’ reactions, behavior and overall perceived value throughout each of the steps in the buying process are altered, for both businesses and consumers alike.</p>
<p>The missed opportunity for the solely internet-based company is to catch the fragmented segments of buyers who may not purchase your product or service through the web. Without a diversified channel strategy, a pure-play web-based company misses revenue opportunities with existing and potentially new customers whose actions are fickle, or as the article would describe them: unfettered.</p>
<p>Comparing the strategies of Amazon to Barnes and Noble in light of the aforementioned analysis should give a clear indication on strategy and its effective execution. First, we know that Barnes &amp; Noble differs from Amazon in its current method of channel distribution. While B&amp;N distributes <a title="advertising online and in store" href="http://www.deploid.com/blog/cloudvertising/">both online and in-store</a>, we know Amazon has no storefront presence. This fact alone helps to shape the strategies of both companies.</p>
<p>Unlike Barnes &amp; Noble, Amazon has a limited channel approach. However, they seem to make up for the limit in channels by offering more than that simply books. While books remain Amazon’s flagship product, the company is diversified as to the depth of its offerings, giving them the ability to cross-sell and upsell while people are visiting their virtual store. They intend to keep unfettered online customers engaged by giving them more options when it comes time to re-up and purchase more.</p>
<p>Barnes &amp; Noble, on the other hand has a more expanded channel and a limited product offering (in this case, mostly books). This allows them to eliminate fickle online defectors by offering a physical location. In addition, the company is able to directly address the needs of its customers by having customer needs addressed by face-to-face contact at physical locations. The example given in the article of Nordstrom and their customer-focused approach is something Amazon could not even attempt to do. However, the inviting atmosphere of a Barnes &amp; Noble store does much more for the needs of its customers than Amazon.com’s website does for theirs.</p>
<p>Increasing the number of channel options is a competitive advantage for Barnes &amp; Noble as they are able to capture and corral online buyers. Both strategies are relevant given the respective companies’ core competencies. One company that would be interesting to look at, who may be doing a little of both is Wal-Mart.</p>
<p>The company most likely to reap the rewards of zero defections is Amazon. Amazon will be able to retain customers for longer periods of time than Barnes &amp; Noble. Unfortunately Barnes &amp; Nobles expanded channel approach does not make up for the grossly expanded product approach strategy Amazon has taken. Customers are much more likely to be retained and purchase additional products from Amazon because they offer such a variety. B&amp;N, on the other hand, really is only intent on targeting those customers who are looking for reading materials which limits their retention of customers because customers are not just about reading.</p>
<p>Barnes &amp; Noble, however, does fulfill more customer needs in an emotional sense than does Amazon. Amazon does not have any face time with actual customers and therefore does not fulfill some of their basic needs for human-to-human interaction. Attempting to make up for this by offering product reviews and interactive features on the website is somewhat helpful, but will never compare to a friendly in-store clerk.</p>
<p>However, when it comes to profitability, long-term retention is key to higher profits per customer. When it comes to the best ability for longer retention of customers, Amazon beats Barnes &amp; Noble.</p>

However, one thing that technology has done to hospitals is that it has helped to streamline the way patients are managed when they come to visit the facility. There are a few specific technologies which have really helped in the recent streamlining of hospital operations.</p>
<p>The first technology that has been helpful to hospital management has been <a title="digital signage for hospitals" href="http://www.deploid.com/markets/medical/">hospital digital signage</a>. This particular technology was once mostly used for advertising and while it is still helpful as an advertising tool in healthcare, it is also currently used as an educational mechanism, giving companies the unique ability to educate and inform patients who are usually sitting for long times in the waiting room. Having such technology may help to actually decrease the perceived wait time for people who are visiting the hospital and waiting for their next appointment.</p>
<p>Perhaps the greatest technological advance in healthcare in the last 30 years has been the advent of personal computing. Oh, how it has helped to streamline the process! Hospitals can not only keep track of patients, but everything is now done in digital form, that includes x-rays and all documentation of patient records. In fact there are those bleeding edge doctors who actually are now using iPads to input patient data and directly upload it to their hospitals servers. Of course, the patient privacy act has really put a lockdown on what can be shared, but this is easily controlled with the amount of security features that can be included in today&#8217;s computing technology.</p>
<p>Where will we be in the next tens years of computing in hospitals? It certainly still remains to be seen, but we will certainly continue to see positive shifts in how technology is changing the way hospitals operate.</p>

<p>You can almost see yourself getting lost in translation, sifting through the sources of such a service for your hospital or healthcare facility. <p>In addition, there are many medical and healthcare facilities which have not updated their archaic files from analog or magnetic to digital formats. <p>Yet another important issue with medical records is that you never know when you will need them on-demand. Having this type of data backed up on the cloud may be essential to the survival of an individual person. The need is there and it will continue to grow as immigrants and visitors need healthcare&#8211;especially those of the more aged generation.</p>
<p>Certainly most of the drug companies who are now pushing their products on to the consumer will have full reign over translating their promotional, informational and warning materials. But, those who actually run the hospital often find that because they are the ones dealing directly with patients, customers and staff that they are the ones who need translation services most often because of the differences in languages many of the people speak who enter these hospitals.</p>
<p>This is just one of the many reasons hospital and healthcare costs are increasing. Wouldn&#8217;t it be nice if hospitals could put this on their priority list as something that was a top priority so costs would actually decrease instead of increase.</p>
